DECLARATION-CONSENT FOR PARTICIPRATION IN THE PROGRAM “LOYAL CLIENTS”

Accepting the present declaration, I give my consent to “Philly Vibe” AD to process my personal data for the purpose of registration and participation in the Program “Loyal Clients”, in which I shall have the possibility to receive awards, special promotions, etc.

I am aware that with providing my personal data I became “personal data subject” and “Philly Vibe” AD – “Personal Data Controller” according to the requirements of the GDPR.

I give my consent “Philly Vibe” AD to collect and process my following personal data:

• Name
• Email
• Phone number

I am aware that the Processor is not allowed to use my data for other purpose and shall not transfer my data to third parties.

I give my consent freely and informed for the purposes as stated in this declaration.

I am aware that I can withdraw the consent given at any time by contacting the Controller at the following email address: help@pizzalab.bg, after which the Administrator can no longer use my data for the Program. The withdrawn consent will not affect the processing of the data for other, legitimate purposes, such as for tax purposes.
